Application/Enrollment Checklist for Montgomery College
Please include all items when you are submitting your application, and make sure all the forms are signed to avoid delay in processing your registration.
- Your Army VOTEC Enrollment Form.
- Montgomery College Registration Form
- Employee's In-County Tuition Rate Agreement *This form will also require the signature of your immediate supervisor.
- MC Authorization to Release Grades Form
- You must contact Sherralyn Bassey, at Montgomery College, Phone: 240-567-1819 before submitting your completed application.
There are three ways to submit the completed application/enrollment packet to Montgomery College: fax, scanned and sent as an e-mail, or by mail:
