Sample Documentation Guidelines

Guidelines concerning documentation should include the following:

  1. Include a statement indicating the current status of the disability and the relevance of the disability to the institutional setting. It would be preferable for individuals who are seventeen years of age or older to be documented as adults. Individuals who are coming directly from high school should have documentation that is no more than three years old.
  2. Provide documentation by a professional who is knowledgeable about the disability.
  3. Documentation includes recommendations for academic accommodations.
  4. Question students at this time as to why they are requesting specific accommodations.
  5. Have documentation support requested accommodations.
  6. Encourage county school systems to have the last triennial reviews at least include a letter addressing accommodations.
  7. Have postsecondary schools write letters to transferring schools outlining the types of accommodations used while at their institution.
  8. Documentation must be kept for the period of time required by the Records and Registration Office (often seven years).
  9. Have an archiving procedure for inactive files containing confidential documentation.
