Disability Access Statements:
Requests for Accommodations

Entities of the institution should be encouraged to include statements about the availability of accommodations on materials as part of an effort to encourage full participation in all programs and services.

Statements about the availability of accommodations are a way of inviting individuals with disabilities to participate in creating a more accessible campus. By encouraging advance inquiries, you can identify any needed accommodations early so they can be readily implemented in a timely manner.

All programs/events at the institution must be accessible. When planning conferences, events, and activities, designate someone to be responsible for handling requests for accommodations. For registration brochures, invitations, or fliers, use an access statement such as:

  • "To request accommodations for a disability, please contact [name, department, address, phone number] at least 2 weeks before the [event/workshop]."
  • "For disability-related accommodations, please contact [name, department, address, phone number] at least 2 weeks before the [event/workshop]."

Publications, such as course syllabi, college bulletins, program brochures, class schedules, newsletters, and instructional publications must be provided in alternative formats (Braille, large print, tape, computer disk) upon request; [name and phone number of office] can provide assistance with document conversion. In these publications please use one of the following statements:

  • "This publication/material is available in alternative formats upon request. Please contact [name, department, address, phone number, e-mail]."
  • "If you need to request this information in an accessible format (Braille, digital, tape or large print) please contact [insert name and contact information including an e-mail or TTY number]."

Web Pages should include a statement to appear as text on pages if you are not sure of usability:

  • "If you have difficulty accessing any portions of this webpage with assistive technology, please contact [insert page owner/webmaster name, department, phone number, e-mail]."
