Course Syllabus Statements

The following are some sample statements for faculty to include in their course syllabi and announce to all students during the first day of class:

  1. Any student who may need an accommodation due to a disability, please make an appointment to see me during my office hours as soon as possible. A letter from Disability Support Services authorizing your accommodations will be needed.
  2. Any student eligible for and needing academic adjustments or accommodations due to a disability is requested to speak with the professor no later than (date).
  3. If you have specific physical, psychiatric or learning disabilities and require accommodations, please let me know early in the semester so that your learning needs may be appropriately met. You will need to provide documentation of your disability to Disability Services in (provide department, building and room).
  4. I encourage students with documented disabilities, including non-visible disabilities such as chronic diseases, learning disabilities, head injury, attention deficit/hyperactive disorder, psychiatric disabilities, to discuss with me, after class or during my office hours, appropriate accommodations.
  5. Students with documented disabilities who may need academic accommodations should discuss these with their professors during the first two weeks of class.
  6. If you need an accommodation based on the impact of a disability, you should contact me to arrange an appointment as soon as possible. At the appointment we can discuss the course format, anticipate your needs and arrange for accommodations. I rely on the Office for Disability Services for assistance in verifying the need for accommodations and developing accommodation strategies. If you have not previously contacted the Office for Disability Services, I encourage you to do so.
  7. This university abides by Section 504 of the Rehabilitation Act of 1973 that stipulates no student shall be denied the benefits of an education "solely by reason of a handicap." Disabilities covered by law include, but are not limited to, learning, psychiatric, hearing, sight and mobility disabilities. If you have a documented disability that may have an impact on your work in this class and for which you may require accommodations, please see me or the Coordinator of Services to Students with Disabilities, (name and office number) so that accommodations may be arranged.
  8. The college will make reasonable accommodations for persons with documented disabilities. Students should notify the Counselor for Students with Disabilities located in (provide department, building and room) and their instructors of any special needs. Instructors should be notified during the first week of classes.

In addition, a statement about arranging for assistance during an emergency evacuation should be added, such as:

  1. If you will require assistance during an emergency evacuation, please contact me so we can make arrangements in advance.
