Please find below the payment forms needed to submit your Make It Happen! Innovation Grants (MIHIG). Click on the blue underlined report names for an online version of the report.

Payment Forms - Access and print your own copy of the above spreadsheet of payment scenarios and documents needed.

Direct Pay Form - Form used for requesting direct payment.
Direct Payroll Deposit Form - For college employees.
I-9 - Federal Employment Eligibility form.
Letter of Agreement - For paying individuals $1,500 or less.
Long Contract - For paying individuals over $1,500.
Honorarium Confirmation - For honoraria.
MW 507 - Maryland tax form.
Paid Invoices/Receipts/Statements - Please provide the originals!
Petty Cash Reimbursement Form - For amounts <= $50.
Personnel Action Form - Will be filled out and supplied by our office.
Requisition to Procurement - Will be filled out and supplied by our office.
Special Purchasing Request Form - State reasons why a certain individual was chosen for a task.
Transfer Memo - Provided by you, signed by authorized individual.
W-4 - Federal tax form.
W-9 - Federal tax form.
