  PLEASE NOTE:

  Both CIGNA medical and dental plans will now offer dependent coverage to age 25 provided they are a full-time student OR an IRS qualified tax dependent. Kaiser already offers this benefit. If you have a tax dependent who is eligible to be on the Cigna plans, a new enrollment form must be completed. If they are a full-time student, please submit the Cigna affidavit form and the Group Insurance Enrollment Form. If they are not a full-time student, but an IRS qualified tax dependent, you must submit a copy of your most recent tax return (first page only) that shows the dependent listed, along with the Group Insurance Enrollment Form. Please black out the financial numbers.
