Both of the forms below are the same form except one is an electronic version and one is a PDF version, please fill out only ONE form.

DL Information Form (this form MUST be filled out electronically using Microsoft Word)

New Section Request Form for those interested in teaching new online courses. This form is provided by the scheduling office and should be printed on BLUE paper. When completed for a new DL section, please have it approved by the appropriate dean and send it to the scheduling office along with the DL Information Form above.

ESH Request Form - The Distance Learning Course Development Program. This form describes the criteria for receiving ESH for developing online courses.
