program. (You must select one

Submit all
official transcripts to the admissions office.
If transcripts are from a foreign country, they must be evaluated by an
evaluation service. Packets for this are
available in the admissions office.
ð
Make an
appointment with an admissions counselor at Student Services to discuss goals
and arrange for placement exams in English, math, and reading if necessary.
ð
Take applicable
placement exams.
ð
Be eligible for: ð EN 101
ð MA 110
ð
Complete or
receive transfer credit for: ð BI 204
ð PY 102
ð
Priority will be
given to students who have completed BI 204/205, EN 102, and
MA 110 or higher.
ð
Have a GPA of 2.5
or better in most recent 24 credits.
ð
After completing
the above steps, submit your Health Science application with the necessary
documentation to the admissions office in the Student Services.
(The necessary documentation is
listed below.)
LPN:
ð
Submit a written
recommendation from your current supervisor.
Be sure it includes verification of employment in the role of LPN for 1
year fulltime or 2 years part-time at a hospital or long term care facility.
ð
Submit your
Military Medics:
Military Medics with at least one year of in-hospital experience will be
evaluated on a case by case basis.
ð
Submit documented
past or present status of a “Navy Corpsman” or as an “Army 91WM6” or “USAF 4NO”
skill identifier.
ð
Submit
documentation of other required experiences such as administration of
medication, documentation of patient care, experiences with use of medical and
surgical asepsis and maintaining safety in patient care.
ð
Provide
certificates of completion and course outlines as required by the Nursing
Department evaluator.
ð
Take the
Excelsior College Exam #403 Fundamentals of Nursing, 8 lower-level credit
examination and receive a grade of “C” or better the first time it is

Submit your
original Excelsior Exam Certificate of Completion with your application for
admission.
